In order to participate all golfers must have a current physical. The physical form can be found by downloading Wake County athletic participation forms at the following link.

In addition to the physical, all athletic participation forms must be completed via the Dragonfly system. Visit the following link and follow the directions to sign up. Then complete all required information.
